Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

Whilst we make every effort to provide the best service possible to everyone who attends our practice, however we are aware that things may not always happen as smoothly as we would like, resulting in a patient feeling that they have a genuine cause for concern, if this is the case we would welcome the opportunity to address any issues which may be raised so we can work to make these right.

We do value feedback from our Patients, whether in the form of compliments, comments, suggestions or complaints.

Our Practice Manager, Amanda Lloyd, oversees all comments and complaints for the practice, however in the first instance these should be raised with either Adrian or Shannen as Team Leaders. 

To pursue a formal complaint please complete this form, or alternatively you may wish to or email the Practice Manager at Riverside.inbox@nhs.net where the Practice Manager will deal with your concerns appropriately.
